Technology Planning Partnership Agreement
The following agreement, between the National Center for Technology Planning (NCTP) and the State Department of Education (SDE) partner (represented by the name(s) indicated in the agreement) is entered into voluntarily and is evidenced for acceptance by the electronic submission of this form on the part of agency representative(s).
Tenure of agreement: March 1, 2010 - December 31, 2010
Conditions of the agreement:
Assurances by NCTP Assurances by Agency Partner
Provide license to duplicate unlimited copies of the three award-winning books by NCTP Founder, Dr. Larry S. Anderson:
  • Technology Audit Survivors Guide;
  • Guidebook for Developing an Effective Instructional Technology Plan, Version 3.5;
  • Visions & Revisions,

and all other materials where "duplication rights" are indicated in the original letter transmitted from NCTP to agency;

 Receive original copy of NCTP documents;
Provide license to disseminate unlimited copies of NCTP materials where "duplication rights" are indicated in the original letter transmitted from NCTP to agency; Perform all necessary duplication, on site, or via other media, in accordance with agreement reached with NCTP (agency web site, CD-ROM, etc.);
Provide to agency any revision updates during period of this agreement; Notify recipients of allowances and limitations on duplication of materials;
Provide invoice for payment. Pay NCTP within thirty (30) calendar days of receipt of invoice.
By completing the information in the form below, I agree to accept and abide by the stipulations of this mutual agreement. Submitting this form electronically constitutes a binding agreement, just the same as filing the hard copy form transferred from NCTP to the State Department of Education.
